Getting Appointed
Intuitive welcomes business from any professional insurance agent - regardless of size. There are no minimum premium volume requirements in order to be appointed. However, you do need to be appointed in order to place business. Please:
- Complete the attached
Agency Agreement
Profile Form.
- Send a copy of the dec page of agency's E&O policy, and your agency licenses in the states you will be doing business.
- Send a W-9 tax form.
